經皮微創空心L形針刀治療彈響指_《中國修復重建外科雜志》

【摘要】目的探討經皮微創空心L形針刀治療彈響指的療效。方法 2007年9月－2009年9月，采用自行設計的空心L形針刀微創治療160例（202指）彈響指。男47例，女113例；年齡12～68歲，平均55歲。病程2周～1年。拇指58指，示指20指，中指46指，環指60指，小指18指。根據Quinnell分級標準：Ⅲ級63指，Ⅳ級126指，Ⅴ級13指。術中首先用空心L形針刀自皮下潛行至A1滑車部遠端，向遠端平行縱向切割后，自針刀空心向腱鞘部注入倍他米松。結果患者手術時間5～19 min，平均8.2 min；無手術相關并發癥發生。160例均獲隨訪，隨訪時間1年～3年6個月，平均1.6年。術后1周36指手術松解處疼痛，口服非甾體類消炎藥后緩解。術后1周25指屈伸活動時仍存在扳機樣感及頓挫感，1個月后5指恢復正常，20指癥狀無明顯改善；其中10指再次行針刀治療后癥狀緩解，余10指經1～3次針刀治療后無緩解。術后6個月參照Quinnell分級標準評定療效：獲優165指，良27指，差10指，優良率為95.0%。結論經皮微創空心L形針刀治療彈響指安全有效、易于操作。
